The Community Health Center of West Palm was established in Palm Beach County as one of the leading providers of free health care to residents who are unable to withstand the costs of healthcare services.
With over 5300 visits recorded in 2020, the Community Health Center of West Palm has offered basic healthcare services to patients who identify as seniors, children, youth, and individuals from low-income households. At the health center, patients are offered health care services FREE of charge.
Without your continued support, patients who visit the health center would see their conditions continue to deteriorate. Your partnership with the Community Health Center of West Palm helps the clinic to offer year-round clinical services for general health, psychiatry, women’s health, urology, pulmonology, eye (including glaucoma treatment), and chronic disease management.
This type of faith based servanthood has ensured that no patients are turned away or made to believe that they are “undesirable”.
This year, the Community Health Center of West Palm Beach introduced a revolutionary resource for patients, now known as The Community Health Center of West Palm Beach Virtual Care Program. Utilizing telehealth technologies & remote patient monitoring devices, our doctors have been able to see approximately 400+ patients since the launch of these programs.
With your help, this program was launched to virtually provide patients with access to providers at the our center. The virtual care program offers several advantages to patients who cannot take time off of work, possess childcare issues, and lack transportation or the means to travel.
The virtual care program has allowed the care teams at the Community Health Center of West Palm Beach to utilize several bluetooth enabled medical devices, these Remote Patient Monitoring Kits include a digital blood pressure meter, a digital scale & digital pulse oximeter to help patients living with chronic diseases collect their vital signs remotely.
This has given our chronically ill patients a valuable lifeline & offers a powerful extension of our dedicated care to these underserved patients. The Remote Patient Monitoring Program also give our chronic disease management care teams unprecedented insights into these patient's conditions, allowing them to proactively intervene, catch trends & promote healthier habits.
A long-time patient, Diana Pierre, has experienced results in only 1 month in the program! Diana has lost approximately 7 pounds, her blood pressure has dropped to normal, and more importantly, she is engaged in the betterment of her health.
